Program Description and Services

Carver County Adult Mental Health

The mission of the Carver County Adult Mental Health Program is to work collaboratively with individuals experiencing serious and persistent mental illness to help them achieve optimal wellness. This is accomplished through encouraging independence and involvement in community. We connect people with the services and resources they need to reach their life goals.

The Adult Mental Health Program provides adult mental health targeted case management services and community support program services (CSP).

Case Management

Case management services help adults gain access to medical, social, educational, vocational, and other necessary services as they relate to the person’s mental health needs. We partner with participants to complete functional assessments, develop an individual community support plan (ICSP) to identify goals and develop strategies for goal achievement, and provide support in increasing overall health and wellness. A case manager will meet with program participants at least once per month.

Community Support Program

The Community Support Program provides client outreach services, social, recreational and educational activities, housing support, linkages with employment resources, crisis prevention assistance, and medication monitoring. We offer a monthly calendar of activities, which includes drop-in visits.

Admission Information

Eligible individuals have a serious and persistent mental illness, as defined by MN Statute: An adult who has a mental illness and meets at least one of the following criteria:

• The adult has undergone two or more episodes of inpatient care for a mental illness within the preceding 24 months.
• The adult has experienced a continuous psychiatric hospitalization or residential treatment exceeding six months duration within the preceding 12 months.
• The adult has been treated by a crisis team two or more times within the preceding 24 months.

The adult:

1. The adult:
   
   1. Has a diagnosis of schizophrenia, bipolar disorder, major depression, schizoaffective disorder, or borderline personality disorder, and
   2. Indicates a significant impairment in functioning; and
   3. Has a written opinion from a mental health professional, in the last three years, stating that the adult is reasonably likely to have future episodes requiring inpatient or residential treatment, of a frequency described in clause (a) or (b), unless ongoing case management or community support services are provided
2. The adult has, in the last three years, been committed by a court as a person who is mentally ill under chapter 253B, or the adult’s commitment has been stayed or continued;
3. The adult was eligible under clauses (1) to (5), but the specified time period has expired or the adult was eligible as a child under section 245.4871, subdivision 6; and has a written opinion from a mental health professional, in the last three years, stating that the adult is reasonably likely to have future episodes requiring inpatient or residential treatment, of a frequency described in clause (1) or (2), unless ongoing case management or community support services are provided; or
4. The adult was eligible as a child under section 245.4871, subdivision 6, and is age 21 or younger.

A Diagnostic Assessment is needed to determine eligibility for services. Staff may assist in coordinating this appointment, or assist in obtaining records from other agencies.
